These forms will be in effect from June 1st 2025 through May 31st 2026. Please use the following approved forms for children and youth attending your religious formation program and/ or events, young adults or adults that serve as catechists or helpers in your religious formation program or events.
Form A Parent/Guardian/Conservator Permission, Liability Waiver and Medical Information This form must be filled out for each child/youth attending the religious formation program or related events beginning anytime after May 31st.
Form B Parental Consent for youth to Participate and to receive Emergency Medical Treatment This form must be filled for each person attending an event that is away from the parish premises.
Forms A and B For all events away from the parish, the responsible adult who is supervising the youth must have form A & B with him or her at all times during the event. If there is a medical emergency he or she will need to provide these forms to the paramedics.
Forms OA and OB must be used for out of state/country trips/pilgrimages. Any out of state trips must be notarized.
Form D Adult Liability Waiver, Medical Release and Promotional Release Form is to be filled out by all adults and young adults participating in religious formation events or trips. (catechists, helpers, etc.)
